New Patient Consent Forms

Instructions
1. Select "Continue" at the bottom of the consent form below to begin signing the forms.
2. Begin on Page 1 and select "Start" on the left hand side of the page. 
3. Enter initials into the pop up box. 
4. Select "Next" on the left hand side of the page until all initials, signatures, and dates are complete.
5. After completing final input, a box at the bottom will appear, select "Click to Sign." 
6. A box will then appear asking you to input your email address. 
7. Enter email in box and select "Click to Sign."
8. Open email and open email titled: "Please confirm your signature on New Patient Consent Forms" from adobesign@adobesign.com. This can take up to 1-2 minutes.  
9. Select the link Confirm my email address. A copy of the completed forms will be emailed to you and to Mind Matters to be placed into your chart.
